Document gathering checklist
Print this pageGather as much as you can before your free consultation. It's fine if you don't have everything — Eugenio will help you fill the gaps.
Identity
- Valid government-issued photo ID (driver license, state ID, or passport)
- Social Security card or other proof of your SSN
Income — last 6 months
- Pay stubs covering the most recent 6 months (every one — not just the latest)
- If self-employed: profit & loss statements for the same period
- Unemployment, disability, Social Security, or retirement income statements
- Spouse income (if filing jointly or living with spouse)
Tax returns — last 2 years
- Federal tax returns (Form 1040 and all schedules) for the most recent 2 years
- State tax returns for the same years if you filed
- If you did not file: a written statement of why
Bank & retirement statements — last 3 months
- Every checking account statement (all pages, including any blank ones)
- Every savings account statement
- Money market and CD statements
- 401(k), IRA, pension, or other retirement account statements (most recent)
Debts
- A list of every creditor you owe with addresses if you have them
- Most recent statement from each credit card
- Medical bills (or a statement of total owed per provider)
- Personal loans and any 'buy now pay later' balances
- Auto loan statement (most recent)
- Mortgage statement (most recent), including any second mortgages or HELOCs
- Student loan account information
- Any collection letters or judgments received
- Any active wage garnishment paperwork
Assets
- Vehicle title(s) or current registration
- Real estate deed(s) if you own property
- Insurance policies — life, auto, homeowners — and surrender values for whole life
- Recent appraisals or estimates of personal property value if you have them
Other
- Any lawsuit papers you have been served with
- Divorce decrees, child support orders, or alimony orders
- Recent receipts for large purchases or gifts in the last 90 days
- List of any business interests
